Abstract: 

Introduction: An impacted tooth is the one which fails to attain its normal functional position within the expected developmental period. They are associated with various pathologies. Hence, the aim of this study was to determine the pattern of occurrence, presentation, and management of impacted teeth among patients attending Muhimbili National Hospital. Materials and Methods: This was a cross-sectional hospital-based study that included all patients aged≥, 21 years and had impacted teeth. The patients were interviewed using a specially designed questionnaire and later examined clinically and radiographically. Data obtained was analyzed using SPSS version 23. 0. Results: A total of 4, 668 patients comprising of 1938 (41. 5%) males and 2730 (58. 5%) females were attended. Among these, 264 (5. 6%) patients comprising of 152 (57. 6%) males and 112 (42. 4%) females had impacted teeth. The 21-25 years age group was the most (59. 2%) commonly affected. Mandibular third molars were the most (64. 9%) impacted teeth followed by 167 (33. 4%) maxillary third molars. Pain was the most common complaint in 79. 1% of patients. Mesio-angular impaction was the most common angulation in 196 (39. 9%) impacted teeth, followed by vertical impaction in 142 (28. 7%) and disto-angular 103 (21. 0%) impacted teeth. A majority (66. 6%) of impacted teeth were treated by open surgical removal followed by 53 (10. 5%) impactions by closed disimpaction. Conclusion: The occurrence of impacted teeth among the patients attended at MNH was 5. 6%. Mandibular third molars were the most commonly impacted teeth with mesio-angular being the commonest type of angulation of impacted teeth. Pericoronitis was the most common pathology associated with impacted teeth, others included dental caries, ameloblastoma, and dentigerous cysts. Surgical disimpaction was the most frequently used treatment method with good healing results.

Abstract: 

Background and Aim: Most dentists extract wisdom teeth. The complications and costs of these procedures, different ideas concerning the need to remove of these teeth, was the purpose of this study.Methods and Materials: Two hundred patients had at least one impacted wisdom tooth in the mandible. After clinical and radiographic examination, factors such as: smoking, degree of eruption, type of impaction, periodontal pocket depth, and history of pericoronitis were recorded. The results were analyzed with SPSS software and Chi square and odds ratio tests.Results: After 3 years follow-up, 58 teeth (29%) were extracted.No correlation between the history of pericoronitis at the start of the study and incidence of removal was seen.Conclusion: the need for prophylactic removal of mandibular impactions is not warranted. We advise more research in this matter.

Abstract: 

Background &Aim: The presence of supernumerary teeth is not uncommon in the general population and it occurs more frequently in individuals with such familial history. However, it is rare to find multiple supernumeraries in isolated cases with no other associated disease or syndrome. No report of bilateral maxillary and mandibular supernumeraries that are located distal to the third molars such as our case is known. These supernumerary teeth may have harmful effect on the permanent dentition if not removed. Case Report: A case with five bilateral maxillary and mandibular supernumeraries is reported in this study. These teeth were removed in four sessions under local anesthesia.

Abstract: 

Introduction: Odontogenic lesions related to impacted teeth are common findings in routine  dental exams, with the potential to cause tooth pain, displacement, bone expansion, or even bone erosion. Thus, their early detection is important. This study aimed to evaluate the frequency of such lesions based on patient age, gender, and location at the Tehran University of Medical Sciences, School of Dentistry, over five years. Materials and Methods: This retrospective study evaluated records from the oral pathology department, covering a five-year period. The association of odontogenic lesions with impacted teeth was assessed. Data on lesion type, patient age and gender, involved jaw, lesion location, and affected teeth were extracted from patient records. Statistical analysis was performed using SPSS 24. Results: Out of 815 odontogenic lesions, 44. 4% were associated with impacted teeth. The patient distribution was 52. 48% male and 47. 52% female. Lesions were most prevalent in the third decade of life (24. 58%), and third molars (59. 39%) were frequently involved. The posterior mandible (68. 5%) and mandibular third molar (55%) were common sites. Dentigerous cysts had the highest frequency (35. 4%), with odontogenic cysts (developmental) occurring more frequently (70. 7%) than odontogenic tumors. Conclusion: The study underscores a significant association between odontogenic lesions and impacted or unerupted teeth. Abstract: 

Statement of the Problem: Gubernacular canal (GC) is a canal that extends from the follicle of unerupted permanent teeth to the alveolar bone crest filled with remnants of the dental lamina. This canal is thought to guide tooth eruption and be related to some patho-logic conditions. Purposes: This study aimed to determine the presence of GC and its anatomical character-istics in teeth, which failed to erupt normally on cone beam computed tomography (CBCT) images. Materials and Method: This cross-sectional study evaluated CBCT images of 77 impact-ed permanent and supernumerary teeth obtained from 29 females and 21 males. The fre-quency of GC detection, its location in relation to the crown and root, the anatomical sur-face of the tooth from which the canal has originated, and the adjacent cortical table to which the canal opens, along with the length of the GC were studied. Results: GC was observed in 53. 2% of teeth. The anatomical tooth aspect of origin was occlusal/ incisal in 41. 5% and crown in 82. 9% of teeth. Moreover, 51. 2% of GCs opened in palatal/lingual cortex and 63. 4% of canals were not located along the tooth long axis. Finally, GC was detected in 85. 7% of teeth undergoing the crown formation stage. Conclusion: Although GC was introduced as an eruption pathway, this canal is also pre-sent in impacted teeth. This means that presence of this canal does not promise the normal eruption of tooth and the anatomical characteristics of GC may influence the eruption process.

Abstract: 

Background and Aim: Different surgical techniques have been proposed for crown exposure among patients under orthodontic treatment. The aim of this study was to investigate periodontal indices in closed flap operation technique used for impacted teeth crown exposure.Materials and Methods: In this descriptive study (2001-2003) 20 patients (15 females and 5 males), age ranging from 15 to 20 years, were participated. Following the performance of closed flap operation technique, in 12 patients impacted canine and central incisors were exposed and treated orthodontically. Periodontal parameters including sulcular depth, width of keratinized gingiva and attachment loss were recorded. Results: The mean of gingival sulcus depth was 1.50±0.43, the means of keratinized gingival in central incisors and canines were 3.07±0.53 and 2.30±0.42, respectively. In one patient, attachment loss was reported.Conclusion: Considering the optimal sulcular depth and keratinized gingiva, the closed flap operation technique seems suitable.
